بِسْمِ اللهِ واللهُ أَكْبَرُ [اللَّهُمَّ مِنْكَ ولَكَ] اللَّهُمَّ تَقَبَّلْ مِنِّي

In the name of Allāh, and Allāh is the greatest. O Allāh, (it is) from You and belongs to You, O Allāh, accept this from me.
